I believe Spanish naming conventions are usually paternal last name followed by maternal, making it perhaps more appropriate to refer to him as Álvarez, but this is not without exception (notably Pablo Ruiz Picasso).

That's true in general, but very common surnames, usually those ending in -ez, are ommitted for brevity in informal situations.
